Payment Policy

Uninsured Patients- As uninsured (self-pay cash) patient, you pay only $100.00, if payment is made at the time of service and no billing service is required.

This $100 fee does not include any lab tests, medical supply, procedure, vaccinations, injections and dispensed medications. However, 20% discount will be offered for these additional services.

Insured Patients- Please be aware that we accept many, but not all insurances. You are encouraged to contact your insurance company prior to your visit to determine if your visit will be covered or how your visit will be covered. Since your insurance is a contract between you and your insurance carrier, we are not party to that contract. Any service that is not covered by your insurance company, for whatever reason, is ultimately your financial responsibility.

My Urgent Care is not responsible for any errors or omissions. It is the responsibility of the patient to confirm with their health insurance company that this facility is participating prior to seeking treatment.

Please note that any lab services, for non-in-house lab tests, are billed separately by the lab, independently.

After receiving an EOB (Explanation of Benefits) from your insurance and/or a statement from our office, you shall be responsible for any difference between your remaining liability and the payment you made at the time of service.
